4. MindLAB & Athletic Wellness Club Information

Information collected in connection with MindLAB or Athletic Wellness Club programming is used solely to support participation, safety, and communication related to those services. Momentum³ does not provide medical diagnosis or treatment and does not collect protected health information beyond what is voluntarily shared for participation and safety purposes(PHI) as defined by HIPAA. Any medical or health information voluntarily provided is used solely for safety and program administration purposes.
